Horizons Behavioral Health, P.C. makes every effort to assist
you in making an appointment as soon as possible after your
initial call. Our hours
are by appointment only, and we do offer evening and weekend
appointments. Once your appointment has been made you are
responsible for payment of that session unless you provide
24-hour prior notice of cancellation.
What Should I Bring To My First
Appointment?
You should bring your insurance card to your first appointment.
If you do not have insurance, we have a limited number
of slots available on a sliding fee scale. Your fee will
be determined once your income has been verified.
You will have some paperwork to
complete during your first appointment and should arrive
approximately 5 minutes early.
